Immediate Sterility after Vasectomy A 2.5-ml injection of 1/1,000 solution of euflavine given down each vas during vasectomy for sterilization will destroy all sperms within the semen and eliminate the necessity for examining two consecutive specimens of semen for azoospermia after the operation. Consultant Surgeons and Vasectomy In late 1971 410 consultant general surgeons and urologists-74% of a national random sample-responded to a postal survey about vasectomy. Probably about 50,000 vasectomies were performed by surgeons in England and Wales in 1970. The service was largely a private one: 60% of consultants' hospital vasectomies were not done under the National Health Service, and, in addition, 26% of the consultants who worked in relevant specialties performed vasectomies elsewhere (...) than in hospital (usually in private nursing homes). Based again on consultants' estimates, probably 6,000 men in 1970 had their requests for vasectomy turned down by surgeons, or more than one turned down for every 10 done. The most common reason was that patients were "too young."About 90% of the consultants thought vasectomy could normally be performed safely and adequately as an outpatient procedure, yet only 64% said that 90% or more of their hospital vasectomies were done on this basis. Psychological Aspects of Vasectomy In a study to assess the psychosocial and sexual effects of contraceptive vasectomy by questionnaire and personal interview, 10 out of 82 respondents indicated some psychological problem arising from the operation. Five out of seven couples interviewed had histories of marital, sexual, or psychological instability. Screening of applicants for vasectomy is therefore recommended, pre-existing instability being a contraindication to the operation.

Sperm agglutinins in seminal plasma and serum after vasectomy: correlation between immunological and clinical findings. The development of sperm agglutinins in serum and seminal plasma in relation to vasectomy was studied in forty-seven men by testing samples taken before vasectomy and on five occasions during the first year after vasectomy. Thirty additional patients were tested only 1 year after vasectomy. One year after vasectomy, sperm agglutinins in the serum in titres from 4 to about 4000 (...) with seminal plasma. The total number of spermatozoa in a pre-vasectomy ejaculate was found to be correlated with an early immune response and with the titre values after 1 year. The group of patients in whom agglutinins had developed 1 year after vasectomy were found to have significantly larger nodules at the sites of operation than those without sperm agglutinins.

Sperm autoantibodies as a consequence of vasectomy. II. Long-term follow-up studies. Thirty-four out of fifty-two vasectomized men studied previously were studied again about 5 years after vasectomy for sperm-agglutinating and sperm-immoblizing antibodies, and for antibodies to human protamine. Only one man lost his already weak antibody activity, whilst one out of eight men, negative at 1 year, appeared to be positive at 5 years. Slightly higher titres in one of the three sperm antibody tests (...) . This prolonged study shows that sperm autoantibodies formed within 1 year after vasectomy are persistent. Their role in remaining infertility after reanastomosis requires further study.
