181. One year symptom severity and health-related quality of life changes among Black African patients undergoing uterine fibroid embolisation Full Text available with Trip Pro

One year symptom severity and health-related quality of life changes among Black African patients undergoing uterine fibroid embolisation The main aim in the treatment of symptomatic fibroids by various modalities including uterine fibroid embolisation (UFE) is to alleviate symptoms and ultimately improve the quality of life. The efficacy of this modality of treatment in Black African women with significant fibroid burden and large uterine volumes is not clear. The main objective of the study (...) was to examine potential changes in symptom severity among Black African patients 1 year following UFE for symptomatic uterine fibroids in a resource-constrained setting, rated using a validated questionnaire (UFS-QOL). Secondary outcomes examined were changes in quality of life and potential associations with age, parity, uterine volume and fibroid number prior to UFE. Additional interventions after UFE were also recorded.A prospective before and after study of Black African patients undergoing UFE

182. Ineffectiveness of magnetic resonance imaging enhancement to predict fibroid volume reduction after uterine artery embolization Full Text available with Trip Pro

Ineffectiveness of magnetic resonance imaging enhancement to predict fibroid volume reduction after uterine artery embolization Fibroid nonenhancement is considered a relative contraindication to uterine artery embolization (UAE) for symptomatic fibroids. This retrospective study assessed the impact of UAE on nonenhancing fibroids to determine imaging predictors of fibroid shrinkage. All women who underwent UAE for symptomatic fibroids between May 2009 and July 2014 and had follow-up magnetic (...) resonance imaging 6 months after UAE were included. There were 59 fibroids (5 nonenhancing, 54 enhancing) among 18 women aged 40 to 53 (mean 46) years. All fibroids were assessed for size, position, and enhancement on subtraction and apparent diffusion constant (ADC) images. Enhancing fibroids had an average decrease in diameter of 19% ± 3%, not significantly different than nonenhancing fibroids, which decreased 23% ± 6% (P = 0.49). Multiple linear regression with percent change in fibroid diameter

183. Treating symptomatic uterine fibroids with myomectomy: current practice and views of UK consultants Full Text available with Trip Pro

Treating symptomatic uterine fibroids with myomectomy: current practice and views of UK consultants The demand for uterus-sparing treatments is increasing as more women postpone childbirth to their 30-40s, when fibroids are more symptomatic. With an increasing choice of treatment options and changing care-provider profiles, now is an opportune time to survey current practices and opinions. Using a 25-stem questionnaire, a web-based survey was used to capture the practices and opinions of UK (...) consultant gynecologists on the treatment of symptomatic fibroids, including the types of procedure most frequently used, methods used to reduce blood loss, and awareness and acceptability of treatment options, and to assess the impact of gender and experience of the treating gynecologist. The response rate was 22%. Laparascopic myomectomy is used least frequently, with 80% of the respondents using GnRHa preoperatively to minimize blood loss and correct anemia, while vasopressin is most frequently used

Patient-reported prevalence and symptomatic burden of uterine fibroids among women in the United States: findings from a cross-sectional survey analysis To estimate the prevalence of women diagnosed with uterine fibroids and the associated symptom burden in the US.Responses of women aged 18-54 years, who completed an online survey, were analyzed. Data were weighted based on age, education, race, geographic region, income, and propensity score to derive national estimates of the prevalence (...) of women diagnosed with uterine fibroids and associated symptom burden. Weighted means and percentages were reported. Prevalence across age and ethnic groups was examined. Symptom burden among women with and without uterine fibroids was compared using weighted logistic regressions.Of 59,411 respondents who met study inclusion criteria, 7.7% reported receiving a diagnosis of uterine fibroids. Of these, 5,670 women (1,402 in the uterine fibroid group and 4,268 in the control group) were excluded from

185. African genetic ancestry interacts with body mass index to modify risk for uterine fibroids Full Text available with Trip Pro

African genetic ancestry interacts with body mass index to modify risk for uterine fibroids Race, specifically African ancestry, and obesity are important risk factors for uterine fibroids, and likely interact to provide the right conditions for fibroid growth. However, existing studies largely focus on the main-effects rather than their interaction. Here, we firstly provide evidence for interaction between categories of body mass index (BMI) and reported-race in relation to uterine fibroids (...) . We then investigate whether the association between inferred local European ancestry and fibroid risk is modified by BMI in African American (AA) women in the Vanderbilt University Medical Center bio-repository (BioVU) (539 cases and 794 controls) and the Coronary Artery Risk Development in Young Adults study (CARDIA, 264 cases and 173 controls). We used multiple logistic regression to evaluate interactions between local European ancestry and BMI in relation to fibroid risk, then performed fixed

186. Magnetic Resonance Imaging-Guided Focused Ultrasound Surgery for the Treatment of Symptomatic Uterine Fibroids Full Text available with Trip Pro

Magnetic Resonance Imaging-Guided Focused Ultrasound Surgery for the Treatment of Symptomatic Uterine Fibroids Uterine fibroids, the most common benign tumor in women of childbearing age, may cause symptoms including pelvic pain, menorrhagia, dysmenorrhea, pressure, urinary symptoms, and infertility. Various approaches are available to treat symptomatic uterine fibroids. Magnetic Resonance-guided Focused Ultrasound Surgery (MRgFUS) represents a recently introduced noninvasive safe and effective (...) technique that can be performed without general anesthesia, in an outpatient setting. We review the principles of MRgFUS, describing patient selection criteria for the treatments performed at our center and we present a series of five selected patients with symptomatic uterine fibroids treated with this not yet widely known technique, showing its efficacy in symptom improvement and fibroid volume reduction.

187. Blood cadmium and volume of uterine fibroids in premenopausal women Full Text available with Trip Pro

Blood cadmium and volume of uterine fibroids in premenopausal women A number of studies have found associations between heavy metals and uterine fibroids, but the results are inconsistent. Here, we conducted this research to demonstrate the relationships between blood heavy metal concentrations and uterine fibroid volume as well as the rate of uterine fibroid presence. In a cross-sectional study, we collected data from 308 premenopausal women aged 30-49 years in Seoul; uterine fibroids (...) are ascertained by past history of myomectomy and pelvic ultrasonography. In the analytic phase, we first analyzed the presence of the fibroids and the concentrations of heavy metals via logistic regression. In subgroup analysis, we used simple and multiple linear regression analyses to examine the associations between heavy metals and uterine fibroid volume. There was no connection between the heavy metal concentrations and the presence of uterine fibroids, but the odds of women having fibroids were higher
