Priapism secondary to penile metastasis of rectal cancer Metastatic penile carcinoma is rare and usually originates from genitourinary tumors. The presenting symptoms or signs have been described as nonspecific except for priapism. Rectal adenocarcinoma is a very unusual source of metastatic penile carcinoma. We report a case of metastatic penile carcinoma that originated from the rectum. Symptomatic improvement occurred with palliative radiotherapy.

T-Shaped Shunt and Intracavernous Tunneling for Prolonged Ischemic Priapism. Conservative management of prolonged ischemic priapism is rarely effective. Interventions include corporal aspiration/irrigation, injection of vasoconstrictive agents or surgical procedures. We describe a technique that fulfills several important criteria in the surgical management of ischemic priapism in that immediate resolution of ischemic pain is achieved, a wide area, reliably patent shunt is created (...) , the procedure is technically simple and it may be performed with the patient under a local anesthetic.We reviewed the records of 13 patients treated with the T-shunt for whom followup, including erectile function, was available.Records were available for review for 13 men who underwent the T-shunt procedure from April 2006 to January 2008. Magnetic resonance imaging of partial segmental priapism (segmental thrombosis of corpus cavernosum). Partial segment priapism or segmental thrombosis of the corpus cavernosum is a rare urologic condition reported 23 times in the world literature. We present the clinical and radiologic findings of a 22-year-old man who presented 7 days after the onset of symptoms in whom the diagnosis was confirmed with magnetic resonance imaging. He was treated conservatively with a nonsteroidal anti (...) -inflammatory drug and aspirin with full restoration of erectile function. On the basis of previous reports and our own experience, we suggest that the management of partial segment priapism should include diagnosis with magnetic resonance imaging and conservative treatment with analgesics and an anti-inflammatory drug to control symptoms.

Priapism and Penile Gangrene Due to Thrombotic Thrombocytopenic Purpura. A 51-year-old man presented with 4 days of idiopathic ischemic priapism. Partial detumescence was achieved with aspiration and injection with intracorporeal adrenaline. On further evaluation, the patient was diagnosed with thrombotic thrombocytopenic purpura. This was treated with plasma exchange and steroids. The penis was initially observed for 3 weeks, which subsequently became necrotic and infected, requiring (...) debridement. After debridement of penile shaft skin, it became apparent that the entire penis was necrotic, necessitating total penectomy. The wound was closed secondarily 2 weeks later. Gangrene of the penis after priapism is very rare. Priapism associated with thrombotic thrombocytopenic purpura has not been reported previously in published data.Crown Copyright 2010. Ketoconazole and Prednisone to Prevent Recurrent Ischemic Priapism. To our knowledge no standard therapy exists for the prevention of recurrent ischemic priapism. We used ketoconazole and prednisone with dosing titrated according to serum testosterone levels to suppress sleep related erections in an attempt to prevent recurrent episodes.Eight patients with recurrent ischemic priapism were treated with ketoconazole and prednisone. Two patients had sickle cell anemia and 6 had idiopathic (...) recurrent ischemic priapism. Testosterone was measured on initial presentation, and ketoconazole and prednisone dosing was titrated to approximately 200 ng/dl testosterone and based on the presence or absence of recurrent ischemic priapism episodes. The International Index of Erectile Function-5 questionnaire was administered to evaluate for erectile dysfunction. Patients were seen monthly and therapy was withdrawn after 6 months.Mean testosterone before and after treatment was 468 and 275 ng/dl
