4. Locally administered bisphosphonate in hip stem revisions using the bone impaction grafting technique: a randomised, placebo-controlled study with DXA and five-year RSA follow-up. (PubMed)

Locally administered bisphosphonate in hip stem revisions using the bone impaction grafting technique: a randomised, placebo-controlled study with DXA and five-year RSA follow-up. Bisphosphonates have previously been shown to increase the density of impacted graft bone. In the present study we hypothesise that bisphosphonates also reduce early stem subsidence. We examined the effect of locally applied bisphosphonate to allografts on prosthetic micromotion and bone density in femoral stem

5. Transient severe motion during arterial phase in patients with Gadoxetic acid administration: Can a five hepatic arterial subphases technique mitigate the artifact? (PubMed)

Transient severe motion during arterial phase in patients with Gadoxetic acid administration: Can a five hepatic arterial subphases technique mitigate the artifact? Gadoxetic acid (Gd-EOB-DTPA) is a hepatocyte-specific magnetic resonance (MR) contrast agent, which has been increasingly used in recent years. However, it has been reported that Gd-EOB-DTPA related transient severe motion (TSM) is sometimes observed during the hepatic arterial phase of MR imaging, which may influence image quality (...) . Since the hepatic arterial phase of contrast enhancement is used for the diagnosis of hepatocellular carcinoma, it is crucial to obtain a decent arterial phase imaging. The present study analyzed motion in patients receiving Gd-EOB-DTPA, comparing a single arterial phase acquisition to a five arterial phase acquisition to determine whether the multiphase acquisition was able to alleviate the TSM-related hepatic arterial MR imaging artifact. It was demonstrated that the single-phase acquisition

7. Dosimetric comparison of five different techniques for craniospinal irradiation across 15 European centers: analysis on behalf of the SIOP-E-BTG (radiotherapy working group). (PubMed)

Dosimetric comparison of five different techniques for craniospinal irradiation across 15 European centers: analysis on behalf of the SIOP-E-BTG (radiotherapy working group). Conventional techniques (3D-CRT) for craniospinal irradiation (CSI) are still widely used. Modern techniques (IMRT, VMAT, TomoTherapy®, proton pencil beam scanning [PBS]) are applied in a limited number of centers. For a 14-year-old patient, we aimed to compare dose distributions of five CSI techniques applied across (...) Europe and generated according to the participating institute protocols, therefore representing daily practice.A multicenter (n = 15) dosimetric analysis of five different techniques for CSI (3D-CRT, IMRT, VMAT, TomoTherapy®, PBS; 3 centers per technique) was performed using the same patient data, set of delineations and dose prescription (36.0/1.8 Gy). Different treatment plans were optimized based on the same planning target volume margin. All participating institutes returned their best treatment

Developing a five-step training model for transperineal prostate biopsies in a naïve residents' group: a prospective observational randomised study of two different techniques. To evaluate a five-step training model for transperineal prostate biopsies (TPPB) and the differences in terms of the detection rate (DR) and the ease of execution when using either the "fan technique" (FT) or the use of a Free Hand technique (FH).A prospective observational randomised study was conducted from September (...) 2015 to November 2017. Six naïve residents, who underwent the same five-steps training model, were randomly subdivided into two different groups of three residents based on the selected TPPB technique: A (FT) and B (FH). Patient characteristics (age, PSA, prostatic volume, DRE, MRI), intraoperative (operative time, number of samples) and postoperative parameters (histologic, pain) were evaluated in the 2 groups. The overall and stratified DR for PSA ranges and prostate volume (PV), operative time

9. Three-Trocar Sleeve Gastrectomy vs Standard Five-Trocar Technique: a Randomized Controlled Trial. (PubMed)

Three-Trocar Sleeve Gastrectomy vs Standard Five-Trocar Technique: a Randomized Controlled Trial. Bariatric surgery is a treatment for morbid obesity. Different surgical procedures have been described in order to obtain excess weight loss (EWL), but currently laparoscopic sleeve gastrectomy is the most commonly performed procedure throughout the world. Reducing abdominal wall trauma and increasing the aesthetic result are important goals for all bariatric surgeons. We conducted a randomized (...) -the experimental group) compared to group 2 (five-trocar technique-the control group). The secondary endpoints were to evaluate any differences between the two groups concerning post-operative pain and patients' satisfaction with the aesthetic results. There was no difference between the two groups concerning age, sex distribution, weight, and BMI. The rate of co-morbidities was similar in both groups. Operative time was inferior in the control group, but patient satisfaction was better in the three-trocar

10. The Root Membrane Technique: Human Histologic Evidence after Five Years of Function (PubMed)

The Root Membrane Technique: Human Histologic Evidence after Five Years of Function The "root membrane" (RM) is a technique that has become popular among implantologists for placement of immediate implants in the anterior maxilla. To present histologic evidence of an immediate implant placed in the human anterior maxilla, according to the RM technique, and retrieved after five years.A fixture, along with the surrounding tissues, was retrieved from the anterior maxilla of a 68-year-old patient (...) , who had been treated five years earlier with immediate implant placement and RM technique. The specimen was processed for histologic/histomorphometric evaluation. The buccal bone plate was maintained without any resorption; a healthy periodontal ligament was evidenced. The implant showed osseointegration, with a high percentage of bone-to-implant contact (BIC = 76.2%). With regard to the space between the RM and the implant, the apical and medial thirds were filled with compact, mature bone

11. Five techniques for improving outcomes of hyperopic LASIK. (PubMed)

Five techniques for improving outcomes of hyperopic LASIK. To determine whether five changes to our hyperopic LASIK protocol produced better outcomes.Five changes, both technical and surgical, were instituted for the treatment of hyperopia. These five changes were nomogram refinements accounting for accommodation, use of a 7.0-mm optical zone and a 9.5-mm transition zone, a targeted mean flap diameter of 10.5 mm, sequential interruption of the laser ablation, and cleaning of the interface (...) (BSCVA) of 20/20 gradually increased to 93% by 3 months. The number of eyes that achieved 20/15 increased by 11.7% compared to preoperatively. Eighty-eight percent of eyes maintained or gained lines of BSCVA. Four patients lost 1 line of BSCVA. One patient lost >1 line of vision due to visually significant microstriae. The outcomes support the observation that five surgical and technical modifications to the hyperopic LASIK procedure result in excellent visual quality and refractive outcomes and a low

12. Percutaneous techniques versus surgical techniques for tracheostomy. (PubMed)

Percutaneous techniques versus surgical techniques for tracheostomy. Tracheostomy formation is one of the most commonly performed surgical procedures in critically ill intensive care participants requiring long-term mechanical ventilation. Both surgical tracheostomies (STs) and percutaneous tracheostomies (PTs) are used in current surgical practice; but until now, the optimal method of performing tracheostomies in critically ill participants remains unclear. We evaluated the effectiveness (...) and safety of percutaneous techniques compared to surgical techniques commonly used for elective tracheostomy in critically ill participants (adults and children) to assess whether there was a difference in complication rates between the procedures. We also assessed whether the effect varied between different groups of participants or settings (intensive care unit (ICU), operating room), different levels of operator experience, different percutaneous techniques, or whether the percutaneous techniques
