throughout the life-course: low birth weight and fetal exposure to androgens; postnatal rapid weight gain; precocious adrenarche and early age at pubertal development; adult weight status and lifestyle. Until recently, candidate gene studies have been underpowered leading to poorly reproducible results. The advent of large-scale GWAS with their stringent statistical thresholds has brought robust new insights, although as yet these have been limited to adult PCOS cases and their direct relevance (...) distributed in a non-sexual pattern. Mild hirsutism may not be a sign of hyperandrogenemia [ ], but the likelihood of androgen excess is increased when associated with other findings such as menstrual irregularities [ , ]. Moderate or severe hirsutism may be a sign of androgen excess in early postmenarcheal years. In adults, the evaluation and grading of hirsutism can be done using the Ferriman-Gallwey scoring system, which may not be suitable for adolescents (modified Ferriman-Gallwey) [ ]. Adult

fat distribution, skin tags, and acanthosis nigricans. Examination of androgen- sensitive skin areas such as the face, chest, abdomen, and back provide an assessment of the degree of hirsutism and can be used to chart patient progress over time; Ferriman–Gallwey scoring can be done if desired. Modified Ferriman–Gallwey (Fig. 2) scoring evaluates nine areas of the body; a score greater than 8 is indicative of hirsutism (3). Grading systems for acne include quantity (mild, moderate, or severe (...) ), location, and quality (comedonal, inflammatory [including erythematous papules and pustules or nodules], or mixed). Definition Hirsutism Hirsutism is excessive terminal hair that appears in a male pattern (excessive hair in androgen-dependent areas; i.e. , sexual hair) in women. Ferriman–Gallwey score The modified Ferriman–Gallwey score is the gold standard for evaluating hirsutism. Nine body areas most sensitive to androgen are assigned a score from 0 (no hair) to 4 (frankly virile), and these separate scores are summed to provide a hormonal hirsutism score ( ).
