Neonatal ovarian cysts: therapeutic dilemma. Seven cases of neonatal ovarian cysts that presented over the past seven years were studied. Complications included torsion and rupture and usually occurred in cysts more than 5 cm in diameter. Surgical removal, either oophorectomy or cystectomy, was the treatment of choice. Because even cystectomy results in loss of normal ovarian tissue, and because spontaneous regression of cysts less than 5 cm in diameter can occur, a more conservative approach (...) is now proposed. Regular ultrasonography alone is recommended if the cysts are less than 5 cm in diameter, and aspiration of the cysts followed by regular ultrasonographs if the cysts are more than 5 cm in diameter. Operation should be reserved for recurrent cysts or for those with complications. Cysts diagnosed antenatally may be aspirated in utero if there are signs of thoracic compression.

Quantitation of lactoferrin in odontogenic cyst fluids. A competitive enzyme linked immunosorbent assay (ELISA) method was used to measure the concentration of lactoferrin in fluids from 36 odontogenic keratocysts, 15 dentigerous cysts, and 30 radicular/residual cysts. Keratocyst fluids contained significantly higher concentrations of lactoferrin than fluids from the other two types of cyst (p less than 0.001); but the range of values obtained within each group was large. Although lactoferrin (...) represents a larger proportion of the total protein in keratocyst fluids than in dentigerous or radicular cyst fluids, lactoferrin concentration is not an absolute diagnostic marker for keratocysts. Lactoferrin concentration correlated strongly with the numbers of neutrophils present in keratocyst fluids (p less than 0.001), less so for dentigerous cyst fluids (p less than 0.05, and not at all in the case of radicular cyst fluids. Tumour-associated trypsin inhibitor (TATI) in human ovarian cyst fluid. Comparison with CA 125 and CEA. The levels of tumour-associated trypsin inhibitor (TATI), CA 125 and CEA were measured in ovarian cyst fluids from 21 patients. TATI in cyst fluid was immunologically and physicochemically similar to the peptide originally isolated from the urine of a patient with ovarian cancer. Mucinous cysts contained significantly higher levels of TATI than did serous cysts. Immunohistochemically TATI (...) was localized in the apical parts of cells of mucinous ovarian cysts. These results suggest that this tumour-associated peptide is actually produced by a tumour. Like TATI, CEA occurred at higher concentrations in mucinous than in serous cyst fluids, whereas CA 125 was found in higher concentrations in serous than in mucinous cyst fluids. An immunohistochemical study of branchial cysts. Twenty five specimens of branchial cyst from the same number of patients have been examined. On staining with haematoxylin and eosin a consistent finding was that the mural lymphoid follicles were always aligned with their mantle zones towards the luminal epithelium. With conventional staining lymphatic sinuses were noted in 17 of the specimens, but with immunohistochemical staining these structures were apparent in 23 cysts. Their frequent (...) occurrence in branchial cysts supports the theory that these lesions are derived from epithelial inclusions in lymph nodes. Immunohistochemical techniques for a range of other markers, using polyclonal and monoclonal antisera, showed a distribution of lymphoid and non-lymphoid tissue elements, as seen in lymph nodes and, for example, palatine tonsils. The lining luminal epithelium also shared many features in common with the crypt epithelium of tonsils.

Multiple intracranial enterogenous cysts. The case of a 40-year-old woman with increasing ataxia is described. Although the clinical presentation and evoked response studies raised the possibility of multiple sclerosis, further investigation revealed multiple cystic intracranial lesions. Fungal Parasites of the Cereal Cyst Nematode Heterodera avenae in Southern Sweden Fungal parasites of the cereal cyst nematode Heterodera avenae were isolated from four sites in southern Sweden. In all, 15 different fungi were isolated from different stages of the nematode life cycle. Among the egg parasites, Verticillium chlamydosporium was common in young cysts on roots, whereas an unidentified species of Verticillium (Verticillium sp. 1) was the dominating species in cysts from soil (...) , especially if the soll had been stored for 8-12 months. V. chlamydosporium was frequently isolated from eggs in cysts from soil, when analyzed shortly after sampling. Subependymal cysts in normal neonates. We undertook a prospective ultrasound study of subependymal cyst formation in normal neonates. Twenty five of 500 normal Chinese neonates (5%) were found to have subependymal cysts. Most of these (18 of 25) showed no obvious perinatal insult. This study suggests a rather high incidence of concealed intrauterine subependymal haemorrhage in normal term neonates.

Transient sexual precocity and ovarian cysts. Nine girls presenting under the age of 7 years with unsustained sexual precocity are described. Large ovarian cysts were detected by ultrasound in three and laparotomy in one. In two girls the symptoms resolved after surgical removal of the cyst; the other seven had spontaneous remission of symptoms, but in two of these transient breast development and bleeding recurred: further ovarian cyst formation was found in one of these patients. Endocrine (...) studies performed before resolution of the cysts showed raised plasma oestradiol concentrations (64 to 440 pmol/l) in three girls and no appreciable rise in plasma luteinising hormone after gonadotrophin releasing hormone stimulation in two. We conclude that ovarian cyst formation with spontaneous resolution may cause transient sexual precocity in girls, and that ultrasound examination is an effective means of diagnosing and following these patients.

Recent experience with choledochal cyst. This report details an 11-year experience with 17 patients ranging from newborn to 17 years with choledochal cyst. Two distinct groups were noted: an infantile group (mean age: 3 months) with obstructive jaundice identical to biliary atresia and a late onset group (mean age: 9 years) with various combinations of pain, mass, and jaundice. Two patients had cystoduodenostomy performed and both required revision. One of six patients who had Roux-Y (...) cystojejunostomy required revision. All seven patients who had primary cyst excision and two patients who had secondary cyst excision with Roux-Y hepaticojejunostomy have been followed prospectively and have done well. The follow-up period ranges from 1-11 years with an average of 5.8 years. Cyst excision should be performed as a primary or secondary procedure whenever feasible. The rare patients with intrahepatic ductal dilatation (Caroli's disease) are best approached by hepatic lobectomy when possible
