81. Endometriosis: diagnosis and management.

and/or abdominal examination is normal To identify endometriomas and deep endometriosis involving the bowel, bladder or ureter. If a transvaginal scan is not appropriate, consider a transabdominal ultrasound scan of the pelvis. Serum Cancer Antigen 125 (CA125) Do not use serum CA125 to diagnose endometriosis. If a coincidentally reported serum CA125 level is available, be aware that: A raised serum CA125 (that is, 35 IU/ml or more) may be consistent with having endometriosis Endometriosis may be present (...) , consider taking a biopsy of suspected endometriosis: To confirm the diagnosis of endometriosis (be aware that a negative histological result does not exclude endometriosis) To exclude malignancy if an endometrioma is treated but not excised. If a full, systematic laparoscopy is performed and is normal, explain to the woman that she does not have endometriosis, and offer alternative management. Staging Systems Offer endometriosis treatment according to the woman's symptoms, preferences and priorities

82. Similar evolution of pain symptoms and quality of life in endometriosis and non-endometriosis patients undergoing assisted reproductive technology (ART). Full Text available with Trip Pro

Similar evolution of pain symptoms and quality of life in endometriosis and non-endometriosis patients undergoing assisted reproductive technology (ART). Altogether 10% of all women of fertile age suffer from endometriosis, and up to 25% of these women require assisted reproductive technology (ART) to conceive. During ART the process of controlled ovarian stimulation causes high levels of estrogen, which in theory increases the risk of the progression of symptoms related to this estrogen (...) -dependent disorder. Because several case reports have described the worsening of endometriosis during ART we carried out this study to investigate whether controlled ovarian stimulation during ART aggravates symptoms in women with endometriosis in terms of pain and quality of life. This prospective cohort study was based on questionnaires containing the Endometriosis Health Profile (EHP-30) and pain evaluated on the numerical rating scale (NRS). Women aged below 40 years were recruited and divided

83. Gastrointestinal and Urinary Tract Endometriosis: A Review on the Commonest Locations of Extrapelvic Endometriosis Full Text available with Trip Pro

Gastrointestinal and Urinary Tract Endometriosis: A Review on the Commonest Locations of Extrapelvic Endometriosis Extrapelvic endometriosis is a rare entity that presents serious challenges to researchers and clinicians. Endometriotic lesions have been reported in every part of the female human body and in some instances in males. Organs that are close to the uterus are more often affected than distant locations. Extrapelvic endometriosis affects a slightly older population of women than (...) pelvic endometriosis. This might lead to the assumption that it takes several years for pelvic endometriosis to "metastasize" outside the pelvis. All current theories of the pathophysiology of endometriosis apply to some extent to the different types of extrapelvic endometriosis. The gastrointestinal tract is the most common location of extrapelvic endometriosis with the urinary system being the second one. However, since sigmoid colon, rectum, and bladder are pelvic organs, extragenital pelvic

85. Endometriosis Involving the Sciatic Nerve: A Case Report of Isolated Endometriosis of the Sciatic Nerve and Review of the Literature Full Text available with Trip Pro

Endometriosis Involving the Sciatic Nerve: A Case Report of Isolated Endometriosis of the Sciatic Nerve and Review of the Literature Endometriosis is a common gynecological condition which affects 5-10% of women of reproductive age and up to 50% of women with pelvic pain and infertility. The most commonly affected areas are the pelvic peritoneum, ovaries and rectovaginal septum. Isolated endometriosis of the sciatic nerve is very rare. Our patient suffered from worsening right hip and buttock (...) pain with severe exacerbation during menstruation. Several different imaging modalities (ultrasound of her pelvis and right hip, as well as X-rays and computed tomography scans of her right hip and lumbosacral spine) failed to identify any pathology. Magnetic resonance imaging scans of her pelvis revealed a 3.5 cm endometriotic lesion over the pelvic segment of her right sciatic nerve. Following a multidisciplinary discussion, the patient underwent laparoscopic excision of endometriosis

89. Genetics of endometriosis: State of the art on genetic risk factors for endometriosis. (Abstract)

Genetics of endometriosis: State of the art on genetic risk factors for endometriosis. Developments in high-throughput genotyping technology have driven discovery of genomic regions associated with an increased risk of endometriosis. In all, 16 genomic regions have been associated with risk of endometriosis in one or more populations. The latest meta-analysis including 17,045 endometriosis cases identified 14 genomic regions supported by results from multiple studies. No independent (...) associations were identified from direct genotyping of common and low-frequency protein-coding variants. This suggests that the most common genetic factors that contribute to endometriosis risk are located in regulatory DNA sequences and alter the regulation of gene transcription. Evidence from different methods is essential to identify the target genes and transcripts that contribute to altered disease risk. Potential target genes in three chromosome regions showing altered gene regulation include

Endometriosis and endometriosis-associated cancers: new insights into the molecular mechanisms of ovarian cancer development Endometriosis is a fascinating disease that we strive to better understand. Molecular techniques are shedding new light on many important aspects of this disease: from pathogenesis to the recognition of distinct disease variants like deep infiltrating endometriosis. The observation that endometriosis is a cancer precursor has now been strengthened with the knowledge (...) that mutations that are present in endometriosis-associated cancers can be found in adjacent endometriosis lesions. Recent genomic studies, placed in context, suggest that deep infiltrating endometriosis may represent a benign neoplasm that invades locally but rarely metastasises. Further research will help elucidate distinct aberrations which result in this phenotype. With respect to identifying those patients who may be at risk of developing endometriosis-associated cancers, a combination of molecular

97. Cerebral infarction associated with benign mucin-producing adenomyosis: report of two cases. Full Text available with Trip Pro

Cerebral infarction associated with benign mucin-producing adenomyosis: report of two cases. Cerebral infarction associated with a malignant tumor is widely recognized as Trousseau syndrome. In contrast, few cases of cerebral infarction associated with benign tumors have been reported. We present two cases of embolic stroke that seemed to be caused by mucin-producing adenomyosis. The patients were women aged 42 and 50 years old. Both patients developed right hemiparesis and aphasia, and cerebral (...) infarctions were detected in the left cerebral hemisphere. There were no other abnormal findings, except for elevation of CA125 and D-dimer. Trousseau syndrome was suspected in both cases, but whole body examinations did not reveal any malignant tumors. However, uterine adenomyosis was detected in both patients. From our findings and a review of the literature, both mucin-producing malignant tumors and mucin-producing benign tumors such as adenomyosis may cause hypercoagulability and cerebral infarction
